Program Application
Potential care recipients will complete a brief questionnaire to help us identify specific needs and concerns.
Applications are not currently available as we are in the initial phases of fundraising. However, your interest is vital to showcasing the need for donor support.

Care Provider Selection
Program participants may select any caregiving agency or certified personal care provider of choice.
Assistance with care coordination is provided for those who struggle with the process of initiating caregiving services.

Collaboration and Follow-Up
CareGifters coordinates directly with the caregiving agency or certified personal care provider to arrange funding for agreed-up on service hours.
Staying connected is a core component of our program! We conduct ongoing needs assessments and routine check-in calls with all program participants. This allows us follow up on service outcomes and identify important needs as they arise.
